Fred Haise signed copy of the Apollo 13 Mid-Course Correction burn notes, part of the meticulous record keeping that Haise, James Lovell and Jack Swigert had to perform in order to re-enter Earth's atmosphere safely after the Apollo 13 oxygen tank explosion. Notes, including red and black writing, are a modern copy, with Haise handwriting at top in blue felt-tip: ''This artifact records some of the most important events and modified procedures that Apollo 13 performed in order to safely return us to Earth! / Fred Haise / Apollo 13 LMP.'' Page of notes are sourced from the ''Apollo 13 LM-7 Contingency Checklist'', with modifications made by Lovell and Haise in-flight. Single sided page measures 5.5'' x 8'' on flexible card stock.
