Partially-printed contact is filled in with type and lists the rate of pay for his appearance as well as residuals for re-runs and international broadcasts of his episode. Countersigned by a representative of Ralph Andrews Productions, Inc. Contract runs 1 page with standard terms and conditions printed on verso. Koufax signs in blue felt tip. Document measures 8.5'' x 14''. Staple punctures to upper left, punch holes to top edge, creasing and some lines across the upper portion possibly from album mounting, else near fine with bold signature.
