Printed document identifies the cause of death as pnuemocystitis carinii pneumonia due to AIDS, with time and place as 3:13 p.m. on 6 February 1993 at New York Hospital in Manhattan. An embossed seal has been impressed to lower right corner by the City of New York. Single-page document measures 8.5'' x 11''. Fine. With an LOA from Arthur Ashe's widow.
