Jonas Salk signed Polio Vaccine Certificate. Dated 1954, the certificate enrolls a Norman Schwab in the first national polio vaccine trial and reads in part, "The National Foundation for Infantile Paralysis certifies that Norman Schwab has been enrolled as a Polio Pioneer…for taking part in the first national tests of a trial polio vaccine…" Verso includes the patient's school and reads, "Attested in Bowling Green, Nassau, N.Y." Card measures 4.5" x 2.75" and is in near fine condition.
