Ride'' in black ballpoint at bottom, card has Ride's Houston address listed along with her ID elements such as age, height, weight, etc. Date of physical examination is listed as 30 April 1987, very shortly before she ended her NASA career. Card measures 3.5'' x 5.5''. Very nearly separated at center, otherwise good condition. With an LOA from CMG Worldwide who represents the Sally Ride estate.
