Gaunt & Son was awarded to an unknown Nottingham Forest player. Formed in the 1880s, Nottingham Forest won the F.A. Cup in 1898 and 1959, the Premier League in 1978, and the Champions League in 1979 and 1980. Front of medal features a relief of 2 soccer players and reads: ''THE FOOTBALL ASSOCIATION''.
