Document is a photocopy of an original form, used to quickly procure signatures in Ohio as Reagan only had 72 hours to collect 1,000 signatures in order to challenge Gerald Ford in the Republican primary. Document is headlined: ''...the following persons to use my name in their Declarations of Candidacies as Delegates and Alternates to the Republican National Convention for the Presidency of the United States by said National Convention to be held in the year 1976...'' Signed clearly ''Ronald Reagan'' at bottom in black ink; countersigned by campaign manager in blue ink, ''John P. Sears''. Document on one page measures 8.5'' x 14''. Near fine condition.
