Partially printed document is headlined, ''Head Quarters Department of Cumberland'' and appoints a Thomas R. Sinton ''assistant Commissary of Subsistence with the rank of Captain of Kentucky Volunteers''. 2pp. document is held together at upper upper left edge. Page two has additional handwritten notes and signatures of other military personnel involved. Measures 7.5'' x 9.75'' with separation along fold on p. 2 and dampstaining throughout. Very good.
