Dated 24 June 1967 shortly before filming began, document lists Wayne's birthdate, age and sex before providing details of his lung cancer treatment in 1964 along with his current prognosis. Signed ''John Wayne'' at conclusion in ballpoint. Document measures 8.5'' x 11''. Toning and some discoloration, mostly confined to edges. Pinholes along top margin and small tear at upper left. Overall good plus condition. With PSA COA, Wayne's signature graded 8.
