Onassis'' and filled out in type with a few handwritten checkmarks and the word ''No'' written 5 times, along with her handwritten social security number. A typed line to verso reads, ''Change of name via marriage Kennedy to Onassis''. Measures 5'' x 3.25''. Perforated cancellations not affecting signature. Near fine.
